Letter of Identification from the Nursing Officer in Charge of the Hospital/Health Centre/Clinic where applicant is presently employed (for Nurses trained outside Nigeria).
Original Sworn Court Affidavit.
Original Police Report.
Photocopy of the Lost Notification letter.
The sum of Three Thousand Naira (N3,000) only in Bank Draft.
Two recent Passport sized Photographs.
Note:
There is no guarantee that document will be re-issued on supply of above listed items. The Registration Officer shall assess the situation and use her discretion to re-issue Notification where need be.
