Original copy of sworn Affidavit stating details of lost certificate and circumstances of loss.
Photocopy of police report.
Current Practicing licence.
2 current passport photographs - endorsed by the Principal of this Council.
Photocopy of Notification or Certificate of Registration of this Council.
Bank certified draft of N3,000 for Re-issue of each lost certificate.
